Use the fields below to estimate pain burden, functional interference, and management difficulty. Results appear above this form after submission.

Formula Used

This tool creates a weighted composite score from symptom intensity, functional interference, frequency, chronicity, flare burden, relief gap, and body-region spread.

1) Intensity Score
((Average Pain × 0.5) + (Worst Pain × 0.3) + (Least Pain × 0.2)) × 10

2) Interference Score
Average of sleep, mood, mobility, work, self care, concentration, and social impact, then multiplied by 10.

3) Duration Score
(Duration Months ÷ 60) × 100, capped at 100.

4) Flare Score
(Flare Days ÷ 30) × 100, capped at 100.

5) Relief Gap Score
100 - Relief Percent. Lower relief increases burden.

6) Region Score
(Body Regions ÷ 10) × 100, capped at 100.

7) Composite Score
(Intensity × 0.25) + (Interference × 0.30) + (Frequency × 0.10) + (Duration × 0.10) + (Flare × 0.10) + (Relief Gap × 0.10) + (Region × 0.05)

Suggested bands: 0-24 Mild, 25-49 Moderate, 50-74 High, and 75-100 Very High. These bands support tracking and discussion only.

How to Use This Calculator

  1. Enter pain intensity values for average, worst, and least pain.
  2. Estimate how often pain is present and how long it has lasted.
  3. Record flare days, pain relief percentage, and number of painful regions.
  4. Score how much pain affects sleep, mood, mobility, work, self care, concentration, and social activity.
  5. Submit the form to view the result block above the form.
  6. Review the chart and score breakdown for clearer discussion points.
  7. Download a CSV for records or a PDF for appointments.
  8. Use repeat entries over time to compare trends.

Frequently Asked Questions

1. Is this a medical diagnosis?

No. It is a structured tracking tool for symptom burden and daily interference. It helps organize information for self-monitoring or healthcare discussions.

2. What does the composite score represent?

The composite score summarizes pain intensity, interference, duration, flare burden, pain spread, frequency, and relief gap on a 0 to 100 scale.

3. Why include both average and worst pain?

Average pain shows typical burden, while worst pain captures peak severity. Using both gives a more balanced picture than either number alone.

4. What is the relief gap score?

It reflects how much pain remains despite the current plan. Lower reported relief produces a higher burden score in the model.

5. Can I use this weekly?

Yes. Repeating the same assessment schedule can make trends easier to spot, especially after treatment changes or new activity patterns.

6. Why does duration matter?

Longer pain duration can signal higher management complexity. The duration score increases gradually and caps to avoid unlimited weighting.

7. When should someone seek urgent care?

Seek urgent care for severe sudden pain, chest symptoms, weakness, loss of bowel or bladder control, fever, or new neurologic changes.

8. Can this replace validated clinical instruments?

No. It is a practical general-purpose tool. Clinicians may still prefer validated questionnaires for diagnosis, treatment planning, or research use.
