Sliding Scale Insulin Calculator

Build flexible scales for glucose checks and meals. Compare clinician-defined ranges with correction settings easily. See dose guidance, exports, trends, and practical examples instantly.

Important: This page is for education and clinician-configured planning. Actual insulin ranges, carb ratios, targets, and correction factors must come from a licensed medical professional. Do not use this page for emergency decisions, severe hypoglycemia, ketones, vomiting, pregnancy-specific dosing, or type 1 diabetes treatment without clinician guidance.

Sliding Scale Rows

Enter clinician-defined glucose ranges and the insulin units tied to each range.

Example Data Table

These sample rows show how configurable ranges, carb coverage, and correction settings can combine into a final result. Replace them with clinician-approved values.

Scenario Glucose Matched scale dose Carbs Carb dose Correction dose Total dose
Breakfast check 188 mg/dL 2 units 45 g 3 units 1.36 units 6.5 units rounded
Lunch check 242 mg/dL 4 units 60 g 4 units 2.44 units 10.5 units rounded
Dinner check 132 mg/dL 0 units 30 g 2 units 0.24 units 2 units rounded
High correction only 315 mg/dL 8 units 0 g 0 units 3.90 units 12 units rounded

Formula Used

Sliding scale dose = units assigned to the glucose range that contains the current blood glucose value.

Meal dose = carbohydrate grams ÷ carb ratio.

Correction dose = max(0, current glucose − target glucose) ÷ correction factor.

Raw total dose = sliding scale dose + meal dose + correction dose.

Final dose = rounded raw total, then limited by the maximum dose cap.

Use clinician-approved ratios, targets, and dose limits only. This page intentionally lets the user configure those values rather than hard-coding medical advice.

How to Use This Calculator

  1. Enter the current blood glucose reading in mg/dL.
  2. Enter the target glucose, carb ratio, correction factor, rounding step, and maximum dose cap.
  3. Add the clinician-defined sliding scale rows from the treatment plan.
  4. Enter carbohydrate grams for the current meal or snack, or leave zero when not needed.
  5. Press Calculate Insulin to show the result above the form.
  6. Review the matched range, dose components, warnings, and graph.
  7. Use the CSV or PDF buttons to save the calculation summary.
  8. Reset or load the example rows to test different scenarios.

Frequently Asked Questions

1) What does this calculator estimate?

It estimates a configurable insulin dose using a user-entered sliding scale, optional meal coverage, and optional correction logic. The settings must match a clinician-approved care plan.

2) Does it prescribe insulin doses?

No. It performs math from values entered by the user. Dose ranges, carb ratios, targets, and correction factors must come from a licensed clinician.

3) Can I use it for type 1 diabetes?

Not without a clinician-approved plan. Type 1 diabetes requires individualized management, and correction-only logic can be unsafe in some settings.

4) Why is there a carb ratio field?

Some insulin plans include meal coverage in addition to a glucose-based scale. The carb ratio converts carbohydrate grams into meal insulin units.

5) Why is there a correction factor field?

A correction factor estimates how many mg/dL one insulin unit is expected to lower glucose above the configured target. It should only be used when prescribed.

6) What does the maximum dose cap do?

It limits the final rounded result so an unusually large number does not exceed the maximum amount allowed by the user’s configured plan.

7) What happens when glucose is outside every range?

The calculator warns that no matching scale row exists. It will not invent a sliding scale dose beyond the rows you entered.

8) Can I save the result?

Yes. After calculation, use the CSV or PDF buttons to download a structured summary along with the current sliding scale table.
