Use milestone counts observed during your recent tracking window. Corrected age is commonly used for premature babies up to 24 months.

Example Data Table

Profile Age (mo) Adjusted Age (mo) Gross Fine Language Social Cognitive Composite Interpretation
Example A 12.0 12.0 7 6 5 5 5 100.0% Generally aligned
Example B 18.0 16.8 8 7 5 6 6 84.4% Monitor and repeat
Example C 24.0 24.0 12 11 10 8 9 106.0% Strong progress for current band

Formula Used

1) Corrected Age
Corrected Age (months) = Chronological Age − Prematurity Adjustment

Prematurity Adjustment
Prematurity Adjustment = (40 − Gestational Weeks at Birth) ÷ 4.345
This adjustment is applied only when corrected age is selected, birth was before 37 weeks, and age is 24 months or below.

2) Domain Attainment Percentage
Domain Attainment % = (Completed Milestones ÷ Expected Milestones for the age band) × 100

3) Weighted Composite Score
Composite % = Σ(Domain Attainment % × Domain Weight) ÷ Σ(Weights)

Weights Used
Gross Motor 25%, Fine Motor 20%, Language 25%, Social 15%, Cognitive 15%

4) Estimated Alignment Age
Estimated Alignment Age = Adjusted Age × Composite %

The score is an organizational tracking aid. It summarizes observations into a structured comparison, not a diagnosis.

How to Use This Calculator

  1. Enter the child’s chronological age in months.
  2. Enter gestational weeks at birth.
  3. Check corrected age only when it applies.
  4. Add your recent observation window and confidence score.
  5. Count completed milestones for each domain.
  6. Submit the form to generate the chart and domain table.
  7. Review the composite score, suggested check-in timing, and domain notes.
  8. Download CSV or PDF if you want a saved report.

Frequently Asked Questions

1) What does this calculator actually measure?

It compares observed milestone counts with an age-based reference band across gross motor, fine motor, language, social, and cognitive development. It summarizes patterns into a weighted tracking score for easier follow-up discussions.

2) Is this a medical diagnostic tool?

No. It is a structured tracking aid for parents and caregivers. It does not diagnose delays, replace developmental screening, or replace clinical judgment from a pediatrician or specialist.

3) When should corrected age be used?

Corrected age is commonly used for babies born early, especially during the first two years. This tool applies the correction only when selected and when gestational age was below 37 weeks.

4) Why are there weights for each domain?

The weights keep the summary balanced while emphasizing broad functional development. They help combine multiple domains into one composite score without treating every area as identical.

5) What if one domain is much lower than others?

A lower domain can signal that the child may benefit from closer observation, repeat tracking, or professional guidance. One lower area should be interpreted with context, not in isolation.

6) Can milestone counts go above the expected value?

Yes. Some children may exceed the reference band in certain areas. The table will show positive variance and a higher attainment percentage for that domain.

7) How often should I update the chart?

Many families review every four to twelve weeks, depending on age and any concerns. Frequent tracking works best when observations are consistent and documented in the same way each time.

8) Why does the tool show a suggested check-in time?

The check-in timing gives you a simple review rhythm based on the current pattern. It helps turn observations into a practical follow-up plan rather than a one-time snapshot.
