Advanced Fall Risk Assessment Calculator

Score key fall indicators with clear weighted inputs. See thresholds, drivers, graphs, and exports instantly. Make screening workflows faster, cleaner, safer for care teams.

This calculator estimates a structured fall risk score from common inpatient screening factors. Use it to document screenings, support care planning, and summarize patient-specific drivers clearly.

Important: This tool is educational and workflow-oriented. It does not replace clinician judgment, local policy, or bedside assessment.

Calculator Form

The page stays single-column overall, while this form adapts to three columns on large screens, two on smaller screens, and one on mobile.

Formula Used

The calculator sums weighted values from six assessment factors. Each selected option contributes its assigned score to the total.

Total Score = History of Falling + Secondary Diagnosis + Ambulatory Aid + IV / Heparin Lock + Gait / Transferring + Mental Status

Point Map

  • History of falling: No = 0, Yes = 25
  • Secondary diagnosis: No = 0, Yes = 15
  • Ambulatory aid: None = 0, Aid = 15, Furniture = 30
  • IV / Heparin lock: No = 0, Yes = 20
  • Gait / transferring: Normal = 0, Weak = 10, Impaired = 20
  • Mental status: Oriented = 0, Forgets limitations = 15

Interpretation Bands

  • 0 to 24 = Low Risk
  • 25 to 44 = Moderate Risk
  • 45 and above = High Risk

How to Use This Calculator

  1. Enter patient details and the assessment date.
  2. Select the best matching option for each risk factor.
  3. Add mobility notes and prevention plan notes if needed.
  4. Press Calculate Fall Risk to show the result beneath the header.
  5. Review the total score, risk band, factor table, and graph.
  6. Download the report as CSV or PDF for records or handoff.
  7. Repeat the assessment when condition, mobility, cognition, or treatment changes.

Case B Yes (25) No (0) Aid (15) No (0) Normal (0) Oriented (0) 40 Moderate
Case C Yes (25) Yes (15) Furniture (30) Yes (20) Impaired (20) Forgets (15) 125 High

FAQs

1) What does this calculator measure?

It estimates a structured fall risk score using six common screening factors, then groups the result into low, moderate, or high risk.

2) Is this a diagnosis tool?

No. It supports documentation and prevention planning. Clinical judgment, bedside assessment, and local protocols must still guide final care decisions.

3) Why does furniture use score higher than a walker?

Furniture walking suggests unstable compensation during ambulation. That usually indicates greater balance or transfer concern than using an established assistive device.

4) When should I repeat the assessment?

Repeat it after surgery, sedation, medication changes, transfers, new mobility limitations, cognitive changes, or any actual fall event.

5) Can this be used for outpatient screening?

It is mainly formatted for structured inpatient-style screening. Community settings often need additional history, balance, and functional performance measures.

6) What do the exports include?

The CSV and PDF include patient details, selected factor options, total score, risk level, summary notes, and suggested prevention actions.

7) What is the maximum possible score?

The maximum is 125 points, which occurs when every scoring factor is set to its highest-risk option.

8) Can I customize the prevention plan text?

Yes. Enter notes in the prevention plan box before calculating, and those notes will remain visible and export with the report.
