Notes: Triglycerides and HDL thresholds consider active lipid therapy. Blood pressure criterion considers antihypertensive therapy. Glucose criterion considers fasting value, HbA1c at selected threshold, known diabetes, or hypoglycemic agents.

Formulae and Thresholds Used
- Definition: Any 3 or more of the five criteria below.
- Waist circumference (central obesity):
- NCEP ATP III: Men > 102 cm (40 in), Women > 88 cm (35 in).
- IDF Europid: Men ≥ 94 cm (37.0 in), Women ≥ 80 cm (31.5 in).
- IDF South/East Asian, Japanese, Chinese, Korean: Men ≥ 90 cm (35.4 in), Women ≥ 80 cm (31.5 in).
- Custom: Enter male/female cutoffs in cm to override.
- Triglycerides: ≥ 150 mg/dL (1.7 mmol/L) or on drug treatment.
- HDL cholesterol: < 40 mg/dL (1.03 mmol/L) in men; < 50 mg/dL (1.29 mmol/L) in women; or on drug treatment.
- Blood pressure: SBP ≥ 130 mmHg or DBP ≥ 85 mmHg; or on antihypertensive treatment.
- Dysglycemia: Fasting glucose ≥ 100 mg/dL (5.6 mmol/L), or HbA1c ≥ selected threshold (5.7% or 6.5%), or known diabetes, or glucose-lowering therapy.
- Useful derived ratios (informative only): TG/HDL and Waist/Height (target < 0.5).
- Conversions: TG mmol/L = mg/dL / 88.57; HDL mmol/L = mg/dL / 38.67; Glucose mmol/L = mg/dL / 18; Inches = cm / 2.54.

1) Criteria Summary by Set
| Criteria Set | Male Waist Cutoff | Female Waist Cutoff | Other Components (apply to all sets) |
|---|---|---|---|
| ATP III | 102 cm | 88 cm | TG ≥150, HDL low, BP ≥130/85, FPG ≥100 or therapy |
| IDF Europid | 94 cm | 80 cm | TG ≥150, HDL low, BP ≥130/85, FPG ≥100 or therapy |
| IDF South Asian | 90 cm | 80 cm | TG ≥150, HDL low, BP ≥130/85, FPG ≥100 or therapy |
| IDF East Asian / Japanese | 90 cm | 80 cm | TG ≥150, HDL low, BP ≥130/85, FPG ≥100 or therapy |
| Custom | As entered | As entered | TG ≥150, HDL low, BP ≥130/85, FPG ≥100 or therapy |
HDL low = <40 mg/dL (men) or <50 mg/dL (women), or lipid therapy.
2) Interpretation Guide
| Met Criteria Count | Status | Typical Next Steps |
|---|---|---|
| 0–1 | Unlikely | Lifestyle reinforcement, routine follow-up |
| 2 | Borderline | Risk discussion, repeat measurements, lifestyle focus |
| ≥3 | Meets Criteria | Clinical evaluation, risk management, guideline-based care |
This table is informational and not a substitute for professional judgment.
3) Common Conversions
| Analyte | mg/dL → mmol/L | mmol/L → mg/dL |
|---|---|---|
| Triglycerides | Divide by 88.57 | Multiply by 88.57 |
| HDL cholesterol | Divide by 38.67 | Multiply by 38.67 |
| Glucose (fasting) | Divide by 18 | Multiply by 18 |
Waist inches = cm ÷ 2.54. Height inches = cm ÷ 2.54.
4) Sample Scenarios
| Scenario | Inputs (key values) | Count | Interpretation |
|---|---|---|---|
| A | Male, ATP III, Waist 104 cm, TG 210, HDL 38, BP 134/86, FPG 92 | 4 | Meets metabolic syndrome criteria |
| B | Female, IDF SA, Waist 82 cm, TG 140, HDL 55, BP 128/80, FPG 101 | 2 | Borderline: two criteria present |
| C | Male, Europid, Waist 90 cm, TG 130, HDL 52, BP 118/76, FPG 90 | 0 | Metabolic syndrome unlikely |
