The calculator uses the classic six-item Revised Cardiac Risk Index structure.

RCRI Score = High-risk surgery + Ischemic heart disease + Congestive heart failure + Cerebrovascular disease + Insulin therapy + Creatinine above 2.0 mg/dL

Each positive item adds 1 point. The total score ranges from 0 to 6.

Creatinine conversion: mg/dL = μmol/L ÷ 88.4

2) Which inputs actually change the score?

Only six items change the score: high-risk surgery, ischemic heart disease, heart failure, cerebrovascular disease, insulin therapy, and creatinine above 2.0 mg/dL.

3) Does age affect the total?

Age matters clinically, but the classic scoring model does not award points for age directly. This page records age for reporting only.

4) How is creatinine handled?

Enter creatinine in mg/dL or μmol/L. The calculator converts μmol/L to mg/dL by dividing by 88.4, then checks the 2.0 mg/dL threshold.

5) What is considered high-risk surgery?

High-risk surgery usually means procedures associated with greater cardiac stress, often major vascular, intrathoracic, or intraperitoneal operations. Local definitions may differ.

6) Why include METs and emergency status?

They do not change the classic total here, but they remain important context. Including them can improve documentation and clinical communication.

7) Can this diagnose heart disease?

No. It is a risk-estimation aid only. It cannot diagnose coronary disease, heart failure, stroke history, or determine treatment on its own.

8) How should the result be used?
