Formula Used

1) Weight conversion: kg = lb / 2.20462

2) Ideal body weight: Male IBW = 50 + 2.3 × (height in inches − 60). Female IBW = 45.5 + 2.3 × (height in inches − 60).

3) Adjusted body weight: AdjBW = IBW + factor × (Actual BW − IBW)

4) Per administration mode: Dose per administration = min(dose factor × dosing weight, maximum single dose, maximum daily dose / frequency)

5) Daily mode: Daily dose = min(dose factor × dosing weight, maximum daily dose, maximum single dose × frequency)

6) Continuous infusion mode: Hourly rate = min(dose factor × dosing weight × 60, maximum hourly rate, maximum daily dose / 24)

7) Volume conversion: Volume = dose amount ÷ prepared concentration

8) Unit handling: All mass entries are normalized internally to milligrams, then returned in the selected display unit.
