Raw score = Item 1 + Item 2 + Item 3 + Item 4 + Item 5

Raw score range = 0 to 25

Percentage score = Raw score × 4

Percentage range = 0% to 100%

Interpretation guide

  • 0% to 28%: very low wellbeing; prompt support may be needed.
  • 29% to 49%: low wellbeing; further assessment may help.
  • 50% to 75%: moderate reported wellbeing.
  • 76% to 100%: higher reported wellbeing.

FAQs

1) What does the WHO-5 measure?

It is a short self-report tool for recent mental wellbeing. It focuses on positive feelings and functioning during the last two weeks rather than listing symptoms alone.

2) How is the score calculated?

Add the five item scores to get a raw total from 0 to 25. Multiply that raw total by four to convert it into a percentage score from 0% to 100%.

3) What does a score below 50 mean?

A score below 50% is commonly treated as a screening flag for poor wellbeing. It suggests that a more complete conversation or assessment may be appropriate.

4) What does 28 or below mean?

Some studies use 28% or below as a stronger alert threshold when screening for depression-related concerns. It still does not confirm a diagnosis on its own.

5) Can this calculator diagnose depression?

No. It is a screening and tracking aid, not a diagnostic instrument. Diagnosis should be made by a qualified clinician who considers symptoms, history, risk, and context.

6) How often should I use it?

Many people use it weekly, every two weeks, or during follow-up visits. The best schedule depends on why you are tracking wellbeing and who is reviewing the result.

7) Why store assessment history?

Single scores can be influenced by timing and context. A history table makes trends easier to spot, especially after lifestyle changes, treatment, coaching, or stressful events.

8) What should I do if scores stay low?

Consider talking with a licensed mental health professional or physician, especially if low wellbeing persists, worsens, or comes with safety concerns, sleep changes, or functional decline.
