Formula Used

Each domain has six items. The calculator gives 10 points for Yes, 5 points for Sometimes, and 0 points for Not Yet.

Domain Score = Yes Count × 10 + Sometimes Count × 5 + Not Yet Count × 0

Chronological Age = Screening Date − Date of Birth

Adjusted Age = Chronological Age − Prematurity Correction

Prematurity correction is used only when the child is younger than 24 months. The correction equals 40 weeks minus gestational age at birth.

Scores at or below the cutoff are flagged. Scores above cutoff but at or below the monitoring threshold are placed in the monitoring zone.
